Frequently asked billing questions

What will my visit cost?

That depends on your plan. We verify benefits before your first visit so you know your expected copay, coinsurance, or deductible responsibility before you walk in the door.

Do you offer out-of-network billing?

Yes. If we’re not in-network with your plan, we can provide a detailed superbill after each visit for you to submit for reimbursement. Many PPO plans reimburse a meaningful portion of the visit.

Do you offer self-pay?

Yes. Transparent self-pay rates are available. Call 909-798-6200 to confirm current pricing.

What forms of payment do you accept?

We accept all major credit cards, HSA/FSA cards, and debit cards. A card is kept on file for copays and any balance owed after your insurance processes the claim.

What’s your cancellation policy?

Please give at least 24 business hours notice to cancel or reschedule to avoid a late-cancel/no-show fee. We know life happens — just call us as soon as you know.
